Help with PDF files

This is to invite you to nominate any Health Occupations Personnel, individuals, or organizations that have offered extraordinary encouragement and/or support to your Health Occupations Programs. Below are the applications and a description and/or definition of the awards. The applications may be copied as needed. This year we will only be awarding one Recognition Award per school. If you are nominating a facility, please remember to put a contact person's name in your application.

All aspects of ACTE summer conference are important, but none more so than recognizing our peers that have excelled in our field. We plan to continue recognizing our recipients at the Awards Luncheon. To ensure we continue to have outstanding award winners, your help is needed. If you submit a nomination, please advise the nominee and offer to help prepare the dossier. Remember, you will also be responsible for the nominee's poster for display at the luncheon. The applications need to be returned to the address below by May 31, 2008. Individuals will be notified in June of the results.

The Annual Awards Luncheon will be held on Wednesday, July 23, 2008. The cost for the lunch is $15.00. Tickets (a total of 100) will be sold on site at the Conference on Tuesday. Please pay with cash in correct change, if possible. School purchase orders will not be accepted.
