SUBJECT:   FormHog Mechanism to submit Documentation

LISTSERVE MESSAGE DATE:  November 21, 2011

The Missouri Department of Elementary and Secondary Education’s Federal Grants & Resources Section will be utilizing FormHog, a web-based system, to collect documentation from districts again this year.  District superintendents and federal program coordinators will be receiving an email this week from info@formhog.com with instructions on how to access the system and upload the documents.   Read the information and instructions provided on each form.  Any submissions made for this school year prior to Monday, November 21, 2011 are deleted as the system was only in the development phase and the early submissions cannot be utilized in the final product.  If you submitted a letter prior to November 21, 2011, you must resubmit the letters and any attachments.  After November 21, 2011, if you have any questions about what letters are required to be submitted in FormHog, please contact David Welch.  If you need technical assistance regarding FormHog, contact David Welch or Tawanda Edwards at 573-751-3468.
