Subject:  First Steps Data Collection Sheets

A set of data collection sheets has been developed to assist with data gathering and entry for the new web system.  These sheets are fairly basic and follow the design of the web system so that if data is gathered on paper, it can be easily transferred to the web.  These sheets do not necessarily follow the First Steps process, so it is important that service coordinators understand First Steps requirements. 

These sheets are not mandatory, and are posted as a Word document so that modifications can be made.  These sheets are NOT considered a part of the official early intervention record; they are merely a tool to facilitate usage of the web system.  Suggestions for additions/improvement of the sheets can be sent to Mary Corey at mary.corey@dese.mo.gov.
