Currently there are two web locations that have First Steps provider enrollment forms – the mofirststeps.com page through the Central Finance Office and the DESE First Steps Provider Enrollment Forms page. 

It is critical that potential providers use the forms on the DESE page and the instructions on the Step-by-Step Enrollment Information page to complete the provider enrollment process.  The CFO will be linking mofirststeps.com to these pages in the future, but the DESE has identified webSPOE roll-out and development of Family Cost Participation components as the CFO priorities in order to best serve the most families, providers, and agencies, and meet legislative requirements.
