- I HAVE completed 4 years of DESE approved student service experience, and have participated in and successfully completed a 2 year mentor assistance program, and also have successfully participated in the yearly performance based Student Services evaluation program of this district, and have developed and implemented a professional development, and have completed 40 hours of appropriate professional development and documented such hours with my district.
- I HAVE NOT completed 4 years of DESE approved student service experience, or have not participated in and successfully completed a 2 year mentor assistance program, or have not successfully participated in the yearly performance based Student Services evaluation program of this district, or have not developed and implemented a professional development plan this is on file with your district, or have not completed 40 hours of appropriate professional development and documented such hours with my district.
